Dear Applicant:

This correspondence is to inform you that your certification as a Minority/Female Business Enterprise ("M/FBE") will expire in three (3) months. Please complete the re-certification application online by logging on to our website at www.occfultoncountyga.com using your Vendor Self Service (VSS) customer identification number VS0000031674. Once you have logged on for the first time, change your password.  In addition to completing the application you will also need to submit a letter on your company's letterhead stating your desire to recertify along with any changes in location, ownership, management and/or control if applicable.  You are also required to submit your previous year tax return, to include all schedules plus a financial statement that reflects revenues, expenses and capital for the previous year.
